In the intro to Star Wars 1/4: A New Hope, it says Star Wars, Episode IV. Was that always in, or did they add it since the New Episode 1?
It was always in there.
In fact Lucas had Episodes 4-6 in mind as the \"Second Trilogy\". Before making Episode 4 he wrote some concepts like \"Journal of the Whills\" where he described shortly how the emperor gained his power and the fall of the old republic.
Indeed he wasn't sure about the details of Episode 1-3 at this time, but the new Episodes follow this concept.

Nope, they're right. The \"Episode IV\" was added later.
Despite rumors to the contrary, \"Star Wars\" was originally written as a single stand-alone movie. Watch it sometime just thinking of it as a single adventure film. If you don't know about everything else, it wraps everything up nicely; for all you know, the threat of the Death Star is gone, the Empire is pretty much wiped out, big celebration at the end, etc.
