So by trying to format a 1.44 floppy, somehow the FAT got converted to RAW. This means the disk cannot be read or formatted. So how did the floppy get changed to RAW? Used win xp pro and tried to do a standard format.

Actually, NT/2k/XP can *read* RAW, but can't write to it if memory serves. Download a copy of RAWrite (you can find it on any site that has Linux for download) and have it format the disk in RAW. Then format the disk in windows to FAT. It looks to me that windows is mis-reading the file system, and/or you've got the disk in read-only mode.
Either way, if that doesn't work, then chuck the disk. They fail so damned easy, its not even funny.

Floppies, being the most unstable of data storage... either Cosmic Rays or poor manufacturing. Your formatting of the disk exposed a flaw on the media, and XP decided to shove it.

Thats kind of like bending a nail while driving it in, and rather than yank it out and get another one for half a cent, you instead borrow your buddy's Nail Straightener D-Lux 9000, read through the 300-page manual on how to use it, and the end result is a nail that may or may not be safe to use.
