Depending on which research you listen to, any city in a developed country will have between 18 and 30 doctors screwing their patients per one million population.

So, for New York, that'd be between 216 and 400 or so doctors doing the wild thing where it is an offence to do so.

Extrapolate that out over a population of 250 million.
