It's not Three Mile Island, and 3 workers were killed when it happened in 1961. I've never heard of this one before, it was pretty thoroughly covered up, and it's the main reason why there are now multiple control rods inside any nuclear fuel pile in modern reactors.

Heh, I've read about this a few times when I've fallen down Wiki Holes relating to nuclear stuff. It's a fascinating story. The Windscale fire in Britain was another crazy one that doesn't seem to get talked about much.
