I have always been of the opinion that Nancy Reagan was, for all intents and purposes the 2nd woman President of the US, after Mrs Wilson when President Wilson had a stroke. This article makes it clear she was saving his butt early in the first term let alone in the 2nd, when Althzheimers had set in more strongly, by all reports.

Especially in Reagan's last term, when Alzheimer's was probably starting to addle his brain. I'll have to hand it to Nancy though, she was strong willed and protective. She was even called the "Chief Protector" by the press. She even told old Strom Thurmond, who got past the Secret Service by claiming to be Ronnie's close friend, to "get out" of his hospital room, where he lay recovering after the assassination attempt. Also apparently after the assassination attempt, she strictly controlled who got access to her husband. She also occasionally attempted to influence his decisions, although I'm of the opinion she pretty much took control of the presidency after Ronnie recovered from his wounds.
