It would appear that "Chris" missed the rung his left foot was aiming for, and the resulting downthrust of the weight of his body threw off his balance and resulted in a hard landing on his back.
He wasn't being careful, because you will see him miss the rung and never once looked down. I don't know about you, but I would be looking at the first couple of rungs just to be sure I didn't miss it. Also, being near the top of the short ladder, his hands had nothing to hang on to. So in that sense the ladder was unsafe to use at that high of an extension.
