I want to take a form from a sheet of paper and be able to enter text into the fields on the computer then print it out. any clues?

You might be able to download the form you want off the internet and edit it then print it.

Scanner + MS Paint == l33t form filler.
Look into Adobe Acrobat...

Adobe Acrobat does this. You need some L33T coding skills though.

Any scanner with OCR capabilities. This will scan the document and convert it to a Word/Word Perfect type document so you can edit it later. It's not perfect but it works.
